Improve LogicMonitor AMP Workspace creation Form in iCare

Incident creation info in LM AMP workspace creation appears as mandatory fields. At least for MaaS consumers, whom not are interested in creating incidents, creates a lot of confussion becasue they don't see any service available to select, and they preceive they cannot fulfill the request. A improvement would be The form should be asking 1st if they are interested in incident creation, and if that's the case show the selection list for assiongmend group, service, etc....
